There are a few ways that you can get your dog back. The first way is by choosing "Love" near the end of the game. This will resurrect your dog. The alternative to this is only available in the Knothole Island DLC. You can choose to sacrifice someone at Cheet-ur's Crypt on Knothole Island and get your dog back.

ABSTRACT:
A method for getting your dog back by using the Rookwood demon door has been claimed. Several trials have been performed, exhausting virtually all possibilities for using the door to reclaim one’s dog. The dog does respawn to allow one to open the demon door. Sometimes, the dog will be present inside the demon door. The dog cannot be permanently reclaimed using this method.
HYPOTHESIS:
The dog can be obtained by using the Rookwood demon door.
DISCUSSION:
Alex D clearly stated above in this post that the dog will respawn if one opens the Rookwood demon door. I previously tried this, and it worked, but the dog disappeared when I left the area. I wanted to test variants of spawning the dog, opening or not opening the door, going in the door or not going in, etc. I want to exhaust all possibilities before I will concede the door cannot be used to permanently give the dog back.
METHODS:
I performed several different trials to get the dog to spawn. I saved the game outside the demon door. I performed one of the tests below, obtained my results, and then restarted the game without saving. This allowed me several tries to work out what was going on with the dog spawning. The trial methods are:
(1) Spawned the dog; opened the demon door; obtained the potion of life from inside; left.
(2) Spawned the dog; opened the demon door; did NOT go in; left.
(3) Spawned the dog; did NOT open the door; left.
(4) Spawned the dog; opened the door; left; came back; went inside the door.
RESULTS:
In general, the dog DOES respawn when the door asks you to show him tricks from the furry one. When the dog spawns, it is different than the original dog. As noted in my initial post that is now gone, the dog is a "spirit" dog with translucent blue and brown, instead of the traditional white and brown. The dog will do tricks and will retrieve the ball. In memory lane, when you can get the dog to spawn there, the dog will tell you a couple dig spots and help you find the chest.
Regarding trial (1), the dog spawned outside the door, and I opened the door. When I went into the demon door (a.k.a., memory lane) the dog was there, and I could play with it. After leaving memory lane the dog was gone. The dog was there again if I went back into memory lane but would despawn if I left.
Regarding trial (2), the dog spawned outside the door. After opening the door, I tried to leave with the dog immediately. The dog ran back to the demon door after I got about 30 feet away. I chased it down; it completely quit responding, even though my UI still had the commands. It just sat there.
Regarding trial (3), the dog spawned outside the door. I tried to leave immediately, and I saw similar results to trial (2). I would get about 30 feet away, and the dog would run back to the door and quit responding. However, when the door started talking to me again, the dog would again act normal.
Regarding trial (4), the dog spawned outside the door. I opened the door. I left. As in trial (2), the dog ran back to the door and quit responding. Then, I went inside the door. Something buggy is going on here. In trial (1), the dog was in memory lane, reproducably. Now it's not. The only difference between this trial and trial (1) is that in trial (1), I immediately went inside the door. When I went into memory lane in this trial, the dog stats are present in the logbook, and the dog commands can be seen when I'm near the door, but I can't find the dog. After leaving memory lane, there still was no dog.
CONCLUSIONS:
The dog is clearly present to allow you to open the demon door. However, it is unclear why sometimes the dog is present in memory lane and sometimes it is not. I think that it is likely this is some sort of a bug.
In terms of being able to permanently get your dog back, these tests support the conclusion that THE DOG CANNOT BE OBTAINED PERMENANTLY BY USING THE ROOKWOOD DEMON DOOR.
